NESUG's W-9 form

Cancellation Policy: A full refund (minus a $50 processing fee) is permitted if written notice of cancellation is emailed or postmarked by October 25, 2012.  NO REFUNDS WILL BE ISSUED AFTER THIS DATE.  NESUG cannot and will not be responsible for any travel or accommodation monies paid if any conference-related activities are cancelled.  We will make every effort to ensure that people are notified of any cancelled activity as soon as possible.
